Submit the LMN, the itemized receipt, and IRS Form 8889 (HSA) or your FSA reimbursement form to your plan administrator. Approval rates exceed 80% for adjustable bases when paired with a sleep apnea or GERD diagnosis.
Effective discount: equal to your marginal tax bracket — typically 22-32%. On a $2,000 adjustable bed, that's $440-$640 back, stackable on top of any sale price.
